[getdns-api] Timeouts

I see that there are return codes that indicate timeouts (e.g.,
GETDNS_RESPSTATUS_ALL_TIMEOUT and GETDNS_CALLBACK_TIMEOUT) but I don't see
a way to instruct the resolver what timeout to use or determine what
timeout it's using.  I understand that in the asynchronous mode I can just
issue a cancel, but that's not the same as a timeout.  This doesn't seem to
be fully specified.
